Notice of Privacy Practices
Your privacy is important. This Notice explains how your protected health information (PHI) may be used and disclosed and how you can access that information.

How Your Information May Be Used
Your PHI may be used for:
Treatment: Providing psychotherapy services, including EMDR therapy and EMDR Intensives
Payment: Billing and collecting payment for services
Healthcare Operations: Practice management, quality assurance, and administrative functions
Your information may also be disclosed without your authorization when required by law, including:
To prevent serious harm to you or others
In cases of suspected abuse or neglect
In response to court orders or legal proceedings
For health oversight activities
Uses Requiring Your Authorization
Certain uses and disclosures require your written authorization, including:
Psychotherapy notes (in most cases)
Marketing purposes
Any use not otherwise described in this Notice
Your Rights
You have the right to:
Access and obtain a copy of your records
Request corrections to your records
Request restrictions on how your information is used or disclosed
Request confidential communication methods
Receive an accounting of disclosures
Receive a copy of this Notice
EMDR Intensives & Confidentiality
EMDR Intensives involve extended therapy sessions that may include in-depth discussion of personal history and experiences.
All information shared during these sessions is protected under the same confidentiality standards as traditional psychotherapy services.
Telehealth Services
If services are provided via telehealth:
Sessions are conducted using secure, HIPAA-compliant platforms
You are responsible for ensuring privacy in your environment
Cancellation Policy
A 50% non-refundable deposit is required to reserve an EMDR Intensive.
14+ days notice: Deposit may be applied to one rescheduled session
Less than 14 days notice: Deposit is forfeited
Remaining balance is due prior to the start of the processing session.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with:
Sonia Mijares, PLLC
or
U.S. Department of Health and Human Services
You will not be retaliated against for filing a complaint.
Changes to This Notice
This notice may be updated as needed. Changes will be posted with a revised effective date.
